Latent tuberculosis infection and active tuberculosis in patients with psoriasis: a study on the incidence of tuberculosis and the prevalence of latent tuberculosis disease in patients with moderate–severe psoriasis in Spain. BIOBADADERM registry

Abstract: Patients with psoriasis who are exposed to biological treatment appear to be at greater risk for tuberculosis. In Spain, up to 20% of patients with psoriasis who are candidates for biological therapy have LTBI. There continues to be a significant percentage of errors in compliance with clinical guidelines.

“…Our two cases corresponded to de novo infections, and there was no single case of reactivation in our 10 patients with latent tuberculosis infection, who started adequate chemoprophylaxis prior to the initiation of adalimumab treatment. The prevalence of latent tuberculosis infection in our cohort (8.4%) appears to be lower than the 29% reported in other Spanish series, or the 20% in the BIOBADADERM registry …”

“…Results from the study carried out by Sánchez‐Moya and Dauden, which included 144 patients with moderate to severe psoriasis treated with tumor necrosis factor antagonists, showed that 29% of the study population were diagnosed with LTBI based on a positive tuberculin skin test and/or signs of past tuberculosis on chest X ray. Subsequently, in the investigation conducted in the BIOBADADERM (Spanish registry for systemic biologic and non‐biologic treatments in psoriasis), including a total of 1425 patients, we found that the number of patients diagnosed with LTBI before starting biologic treatment was notably decreased, accounting for 163 (11.4%) subjects . Likewise, in our investigation LTBI was found with a prevalence of almost 13% of patients with moderate to severe psoriasis.…”

“…25,26 LTBI was diagnosed by TST in 13Á4% of patients with rheumatic diseases (rheumatoid arthritis, ankylosing spondylitis and psoriatic arthritis) undergoing treatment with infliximab in Spain. 27 The clinical implications of diagnosing LTBI are very important.…”
