Laxatives and the Special Role of Senna

Abstract: Laxatives are classified according to their actions on the gut, and the special role and use of senna in the treatment of constipation are discussed. Standardised senna is the most physiological of all the non-fibre laxatives and is safe when used in appropriate dosage avoiding loose stools. Additional advice on bulk supplement should be given.

“…Thus sennosides are not absorbed in the upper gastrointestinal tract. The β‐glucosidase of enteric bacteria hydrolyze sennosides into rhein mono‐anthrones, which become active when they reach the large bowel . This active drug then induces a rapid increase in the motility of the colon , causing 3 to 10 high‐amplitude contractions that occur 6 to 10 hours after ingestion.…”

“…We treat patients with idiopathic constipation and constipation due to anorectal malformations and Hirschsprung's disease. The choice between osmotic and stimulant laxatives is made according to the condition of the patient . We prefer to use sennosides, a stimulant laxative with particular effect on colonic motility.…”

“…Its main pharmacological components are the sennosides. Sennosides are decomposed into rheinanthrone by bacteria in the colon, and then stimulate Auerbach's plexus to exert their laxative effects (Godding, 1988).…”

“…These are degraded in the lower reaches of the gastrointestinal tract only and act for the most part by inducing a net secretion of fluid into the colon . They are also direct stimulants to nerve endings in the colonic mucosa, increasing motility and enhancing colonic transit . While the active principles may appear in breast milk, the amounts are probably insufficient to cause diarrhoea in a breastfed child .…”
