Abstract:Aims We prospectively studied the efficacy, incidence of adverse drug reactions and withdrawal from leflunomide in an outpatient population with rheumatoid arthritis in a setting of care‐as‐usual. Methods In this prospective case series study, a standard dataset was collected from outpatient medical records, including patient and disease characteristics, data on leflunomide use and adverse drug reactions. Results During the study period 136 rheumatoid arthritis patients started leflunomide. Median (range) fol… Show more
“…Van Roon et al. 2 also showed a similar figure of ∼42%, which is much lower than 71% in our study at 2 years. There were no studies to compare 5‐year survival of leflunomide.…”

“…All studies showed a high rate of gastrointestinal adverse effects, consisting of diarrhoea, nausea and elevated transaminases 2,7,8 . Interestingly, our study did not show any cardiovascular side‐effects.…”

“…Some showed high withdrawal rates for leflunomide. This is illustrated in Table 2, which gives a comparison of our data with a few other studies 2,7,8 …”

“…Our study looked retrospectively at a group of patients in a ‘care as usual’ setting in a day‐to‐day clinical practice. This reflects the true side‐effect profile of patients as opposed to the strictly controlled conditions in clinical trials 2,6 . Most of the previous studies in this setting have shown conflicting results.…”

“…Activated T‐lymphocytes play an important role in the pathogenesis of rheumatoid arthritis (RA). Proliferation of lymphocytes is inhibited by A771726 as they are dependent on de novo synthesis of pyrimidines for their cell division 1,2 …”
Our study suggests a better side-effect profile and a better drug survival for leflunomide than suggested by previous studies with survival comparable to that of MTX.

Use of methotrexate with leflunomide increases the risk of pancytopenia compared with use of leflunomide alone. The haematological toxicity of the combination requires further study.
