Left Atrial Mass Invasion from Pulmonary Neoplasm Extension via the Right Upper Pulmonary Vein Presenting as Ipsilateral Stroke

Abstract: Left atrial invasion by lung cancer via haematogenous pathways is a relatively uncommon but potentially life-threatening event. While several cardiac complications of cardiac involvement have been previously described, the evolution towards cerebral stroke has been rarely reported. In this case report, we describe an atypical case of haematogenous metastatic invasion of the left atrium from pulmonary neoplasm extension presenting as an ipsilateral stroke whose ASCO classification changed during the clinical ma… Show more

“…According to Ballo and colleagues, the evolution towards cerebral stroke has rarely been reported. They described an atypical case of hematogenous metastatic invasion of the LA from pulmonary neoplasm extension presenting as an ipsilateral stroke (14). In the present case, the intracardiac and the pulmonary masses were similar on the microscopic examination.…”

“…8 Invasion of the left portion of the heart may thus cause several life-threatening cardiac complications including pulmonary venous flow obstruction, cardiac tamponade, ventricular arrhythmia, complete heart block, as well as myocardial infarction. 11 Intracavitary metastases may also result in peripheral embolization, mechanical hemolysis, or intracavitary obstruction. 1 Anyway, cardiac inflow obstruction or massive tumor embolization invading vital organs can lead to sudden cardiac death.…”

“…Invasion of the LA may be thus taken into account as a catastrophic event which has not been often approached surgically. 11 As a result, use of cisplatin plus etoposide has been advocated to be effective for poorly differentiated large-cell neuroendocrine carcinomas in adjuvant therapy settings. Nevertheless, these results have been derived from retrospective studies.…”
