Left ventricular hypertrophy in patients with hypertension: importance of blood pressure response to regularly recurring stress.

Abstract: Left ventricular hypertrophy (LVH), a target organ response in essential hypertension, is only weakly related to clinical measurements of blood pressure. To determine whether blood pressure measured under basal or stress conditions more closely determines LVH, we compared echocardiographic left ventricular mass index and relative wall thickness with clinical blood pressure and with 24 hr recordings (home, work, and sleep) in 19 normal subjects and 81 patients with mild hypertension. Only a weak correlation was… Show more

“…In Framingham, LVH occurred in 28% of women over 60 years with a systolic BP of 125-139 mm Hg [29]. Furthermore, evidence now suggests that BP explains only 25% of the variability in left ventricular mass [30]. Obesity and insulin resistance have been implicated in the pathogenesis of non-hypertensive LVH.…”

“…A 24-h ambulatory BP (ABP) (Spacelabs 90207, Redmond, WA, USA) was then obtained from each patient on an active day. 11 The device was programmed to measure BP every 15 min during daytime and at half hourly intervals during night-time, daytime being defined as the period between 08.00 to 22.00. The following spurious readings were discarded by editing software; systolic BP Ͻ diastolic BP, diastolic BP Ͼ160 or Ͻ40 mm Hg and systolic BP Ͼ260 mm Hg or Ͻ50 mm Hg.…”

“…12 An ABP (Spacelabs 90207, Redmond, WA, USA) was then obtained from each patient on an active day. 13 Daytime (08.00-22.00, BP sampling every 15 min) mean systolic ABP of у140 mm Hg and/or mean diastolic ABP of у90 mm Hg defined sustained hypertension.…”
