Leiomyoma of the Foot: Sonographic Features with Pathologic Correlation

Abstract: Leiomyomas arising outside of the uterus and gastrointestinal tract are uncommon, though these benign soft tissue neoplasms have been reported in the lower extremity and foot. To our knowledge, the sonographic appearance of a lower extremity leiomyoma has not been described in the literature. This report involves a case of leiomyoma of the foot and its sonographic imaging features. MR correlative imaging and histopathology are also provided.

“…Plain radiographs typically do not visualize these soft-tissue tumors, although leiomyomas can display radiopaque calcifications 1,2,[9][10][11] or scalloping of adjacent bone on X-ray. [12][13][14] In our cohort, none of the lesions displayed these characteristics.…”

“…[1][2][3] The classification system of leiomyomas are three types based on origin: piloleiomyoma, arising from the arrector pili muscles, angioleiomyoma arising from the tunica media found in blood vessel walls, and genital leiomyomas arising from the smooth muscles of scrotum, vulva, nipple and areola. 6 Angiomyomas have shown a predilection for the lower extremity and may cause symptoms due to weight bearing surfaces and irritation from shoe gear. 6 These masses are often seen in individuals between the ages of 20 and 60, however, have been reported as early as the 2nd decade and as late as the 9th decade as well.…”

“…A subcutaneous tumor accompanied by tenderness can have diverse differential diagnoses, such as angiolipoma, angioleiomyoma, schwannoma, glomus tumor and eccrine spiradenoma. [1][2][3][4] In the clinical practise setting, subcutaneous tumors are differentiated by the patient's age and sex, their clinical course and physical findings, including the size and location of the tumor, and the presence or absence of accompanying pain. In most cases, it is possible to narrow the differential diagnoses using clinical information.…”
