Lengthening of short bones by distraction osteogenesis—results and complications

Abstract: We performed bone lengthening surgery on 12 metacarpals and 14 metatarsals of 15 patients. The mean age for metacarpal and metatarsal lengthening was 14.5 (10-21) and 17.5 (10-25) years, respectively. We used a unilateral or a circular external fixator. The mean healing index of the metacarpals and metatarsals was 1.6 (1.1-2.3) and 1.6 (1.0-2.0) months/cm, respectively. The mean increase in metacarpal and metatarsal length was 17.6 (13-26) and 24.3 (20-30) mm, respectively. The functional scores of the metatar… Show more

“…One of the recent developments to decrease the time that patients spend in the frame is the combined use of external fixator and internal fixation, such as an intramedullary nail. 3,15,16 These can be used sequentially (lengthening and then nailing) or simultane ously (lengthening over the nail). These approaches elimi nate the bulky frame while at the same time protecting the still unstable callus site from fracture.…”

“…Unlike other hand fixators that mostly allow only linear lengthening, 9 one of the principle features of the modified mini-fixator set proposed is its versatility that provides assemblies for lengthening of phalanges and metacarpal bones, deformity correction as well as for interphalangeal skin growth and stretching. 4,5 The modified variant of the mini-fixator provides more rigidity of fixation and reduces wire strain on the short bone as compared with the Ilizarov mini-fixator.…”
