Lessons learned from the Marmara disaster: Time period under the rubble

Abstract: Rescue efforts should continue at least for 5 days after the disaster. Time under the rubble is not an adverse prognostic indicator of survival or renal dysfunction for the patients of crush syndrome, probably because only the victims with mild or moderate injuries can survive under the rubble for longer durations.

“…According to a retrospective analysis conducted after the Armenian earthquake, only 3.5% of casualties were extricated by specialized teams (73). External rescue takes at least 24-48 hours to organize, whereas rescue during this period may be critical for ultimate outcome (73)(74)(75). Hence, people living in disaster-prone regions should be trained on how to contribute to the rescue if they remain uninjured (76).…”

“…There were two particularly catastrophic earthquakes in the last decade: the 1995 Hanshin-Awaji earthquake in Japan, and the 1999 Marmara earthquake in Turkey. [1][2][3][4][5]16 The sufferers of CS showed similarly high mortality rates of 13.4% and 13.6%, respectively. Out of 372 Hanshin-Awaji earthquake patients with CS, 239 had CI of the lower limbs.…”

“…1 Similarly, out of 539 Marmara earthquake victims with CS, 436 had CI of the lower limbs. 16 Thus, the lower limbs appear to be the main CI site during massive disasters.…”

“…This important consensus-building conference, which continues every 2 years under the same organizational structure (http:// cybernephrology.ualberta.ca/Banff), led to the Banff Classification, the worldwide standard for renal transplant biopsy interpretation [7][8][9]. Initially, fax was the primary mode used not only in the Banff classification communications, but also in another important activity of the ARF Commission, the Disaster Relief Task Force under the leadership of Norbert Lameire which, in 1999, successfully intervened in a major earthquake in Turkey, saving hundreds of lives [10][11][12][13][14][15][16][17].…”
