Lethal COVID-19: Radiologic-Pathologic Correlation of the Lungs

Abstract: Ground-glass opacities and consolidations in COVID-19 correlate with multiple pathologic processes, notably diffuse alveolar damage, capillary dilatation and congestion and microthrombosis.-Acute bronchopneumonia is more frequently associated with bronchial wall thickening and consolidation.-Vascular alterations such as vascular enlargement sign, capillary dilatation and congestion and microthrombosis are prominent findings in both CT and histopathology. “…Few studies have correlated imaging findings in severe cases of COVID-19 pneumonia with histopathological findings [ 162 , 163 , 164 , 165 , 166 , 167 ]. This issue was investigated especially in case reports and case series mainly based on CT features described in ante-mortem or post-mortem examinations [ 163 , 164 , 165 , 166 ].…”

“…This issue was investigated especially in case reports and case series mainly based on CT features described in ante-mortem or post-mortem examinations [ 163 , 164 , 165 , 166 ]. CT imaging findings such as GGO and consolidations usually correspond to histopathological examinations with diffuse alveolar damage (DAD) in different stages [ 162 , 163 , 164 , 165 , 166 ]. DAD is usually a nonspecific response of the lung to a multitude of injurious agents characterized by edema, endothelial and alveolar injury with hyaline membrane formation in the acute phase, and fibroblast proliferation and interstitial fibrosis in the organizing phase [ 162 , 163 ].…”

“…Recently vascular changes other than PE have drawn attention [ 23 ]. Additional knowledge is, however, required and not yet available to confirm and better understand early observations.…”

“… 1 At the time our article was published, there was a lack of scientific data that correlated chest CT imaging to postmortem pathologic findings in this disease. Recently, Henkel et al 2 published a series of 14 patients who died of COVID-19, in whom a morphologic comparison of antemortem chest CT scans with postmortem gross findings and histopathologic findings was performed. Five of 14 patients in their study also underwent contrast-enhanced CT imaging.…”
