Abstract

Psychosis: case reportA 56-year-old man developed psychosis during off label treatment with levacecarnine for improving his immune system and energy levels during the COVID-19 pandemic [routes, dosages and time to reaction onset not stated].The man presented to hospital due to disorganized thought process, racing thoughts and paranoid delusions of drones following him. He was admitted. On admission, a head CT did not reveal any abnormalities. Laboratory tests including rapid plasma regain, vitamin-B12, HIV, folate and urine drug screen did not reveal any medical cause of his psychiatric condition. He reported that he had brought an online supplement containing high doses of levacecarnine [acetyl-L-carnitine] and ingredients of multivitamin to improve his immune system and energy levels during the COVID-19 pandemic. His symptoms had started a few days after taking the supplement. However, the true ingredients could not be confirmed as the supplement not being approved. Before using levacecarnine, he was functional and running his own health-related business.The man was treated with risperidone and was discharged home after return to his baseline level of functioning.
