LGI-1 encephalopathy following ChAdOx1 nCov-19 vaccination

Abstract: A 71-year-old male patient was diagnosed with LGI1 encephalopathy 4 weeks following a first ChAdOx1 nCov-19 vaccination. Extensive work-up including analysis of CSF and PET examination did not reveal a tangible cause so that a vaccine-associated encephalopathy was considered as differential diagnosis. Under steroid treatment, the faciobrachial dystonic seizures subsided.

“…Whilst the 3 perceived episodes of palpitations captured on ECG showed sinus rhythm, autonomic dysfunction has been well documented in anti-LGI-1 AE. 5,6,11 Indeed, Lewis et al 5 have described a case of anti-LGI-1 AE following the mRNA Pfizer vaccine that presented with sporadic heart palpitations, whilst cognition was affected months later. Further to this, Garibashvili and Heckmann 6 detailed cardiac pauses as a presenting complaint several days before the onset of FBDS following administration of the viral vector ChAdOx1 vaccine.…”

“…2 Cases of anti-LGI-1 AE have now been described following immunisation with the COVID-19 mRNA (BNT162b2/Pfizer & CX-024414/Moderna) and viral vector (ChAdOx1-S/Astra-Zeneca & Ad26.COV2.S/Johnson & Johnson) vaccines. 3-7 Here we describe a 50 year old man who developed anti-LGI-1 AE following a mixed vaccination regimen.…”
