Background: Iran's population is aging. One of the most common problems in Iranian public health is the high prevalence of cardio-metabolic risk factors. The purpose of this study was to investigate the relationship between cardio-metabolic and demographic risk factors and disability in people 60 years and older in Iran.Methods: Data from the Bushehr Elderly Health (BEH) program was used in the analysis. Demographic characteristics, as well as the history of diabetes and other chronic diseases, and smoking were measured using standardized questionnaires. Anthropometric measurements and laboratory tests were performed under standard conditions. Dependency was determined by the questionnaires of basic activities of daily living (BADL) and instrumental activities of daily living (IADL) using Barthel and Lawton scales respectively. Multiple logistic regression was used in the analysis.Results: Mean (Standard Deviation) of the participants’ age was 69.34 (6.4) years (range: 60 and 96 years), and 48.06% of the participants were men. After adjusting for potential confounders, being older, being female (OR (95%CI): 2.33 (1.89 - 2.86)), having a lower education level, a history of diabetes mellitus (OR: 1.43 (1.17 -1.74)) and past smoking (OR: 1.25 (1.00 - 1.55)), and no physical activity (OR: 1.54 (1.23 - 1.87)) were significantly associated with dependency in IADL. Also, being older and female (OR: 2.38 (1.85 - 3.03)), having a lower education level, no physical activity (OR: 2.17 (1.61 - 2.86)) and low daily intake of calories were associated with dependency in BADL.Conclusion: Age, gender, education level and physical activity are risk factors of the dependency in both of BADL and IADL in the elderly people. Diabetes mellitus and past smoking are risk factors of the dependency in IADL, and low HDL cholesterol and low daily intake of calories are risk factors of the dependency in BADL.

Background: Falls are common among elderly adults, and are predictors of hospitalization, institutionalization and mortality. Objective: The objective of the present study was to examine the relationship between blood-based markers of inflammation and fall events in a sample of elderly Hispanic adults. Method: Data were collected from 190 participants enrolled in the Panama Aging Research Initiative study who completed baseline clinical and cognitive assessments. A non-fasting blood sample was obtained. Self-reported falls were classified as no falls, single falls or recurrent (two or more) falls reported in the 12 months prior to baseline evaluations. Serum levels of C Reactive Protein (CRP), T-lymphocyte secreting protein (I-309), interleukin 10 (IL-10), interleukin 6 (IL-6) and interleukin 7 (IL-7) were measured. Global cognition was assessed with the Mini Mental State Examination and depressive symptoms were assessed with the Geriatric Depression Scale (GDS-30). Multinomial logistic regression was used to assess the link between inflammation and fall events. Results: Depressive symptoms, limitations in Instrumental Activities of Daily Living (IADL), IL-7 and I-309 were significantly related to fall events. Elevated levels of IL-7 increased the likelihood of single and recurrent falls, while increased levels of I-309 were associated only with recurrent falls. Greater IADL limitations and depressive symptoms were associated with an increased likelihood of recurrent falls. Conclusion: There is a lack of research investigating the relationship between inflammatory biomarkers and fall events. These results provide evidence of risk factors for falls in Hispanic older adults, and could serve to guide public health professionals to establish clinical guidelines to reduce fall risks.

Adapun tujuan penelitian ini untuk mengetahui hubungan faktor demografi dan demensia dengan kemandirian lansia dalam memenuhi aktivitas dasar dan instrumental di panti wredha. Desain penelitian ini adalah cross sectional. Alat pengumpulan data menggunakan kuisioner BADL (Basic Activities of Daily Living), IADL (Instrumental Activities of Daily Living), dan MMSE (Mini Mental State Examination). Sampel penelitian berjumlah 166 lansia berusia > 60 tahun yang tinggal di 3 panti wredha Bandung dan Garut. Analisa data menggunakan uji independent sample t-test. Hasil penelitian menunjukkan lansia yang ada di panti wredha 65,1% mandiri dalam BADL dan 51,8% mandiri dalam IADL, 109 orang (65,7%) mengalami demensia, 118 orang (71,1%) adalah wanita, 142 orang (85,5%) berpendidikan rendah, dan 153 orang (92,2%) tidak menikah. Analisis menunjukkan terdapat hubungan yang signifikan antara kemandirian (BADL dan IADL) dengan pendidikan, status marital, dan demensia pada lansia di panti wredha (p<0,05). Upaya yang komprehensif perlu dilakukan untuk meningkatkan kemandirian lansia khususnya yang ada di panti wredha. The research sample of 166 elderly aged > 60 years who live in 3 nursing homes in Bandung and Garut. Data analysis uses an independent sample t-test. The results showed that the elderly in nursing homes 65.1% were independent in BADL and 51.8% were independent in IADL, 109 people (65.7%) suffered dementia, 118 people (71.1%) were women, 142 people (85.5% ) have low education, and 153 people (92.2%) are not married. The analysis showed that there was a significant relationship between BADL and IADL with education, marital status, and dementia in the elderly in nursing homes (p <0.05). Comprehensive treatment needs to improve the independence of the elderly especially in nursing homes.
