Limitations in ROP Programs in 32 Neonatal Intensive Care Units in Five States in Mexico

Abstract: Retinopathy of prematurity (ROP) is the main cause of avoidable blindness in children in Mexico despite National ROP Guidelines and examination of preterm infants being a legal requirement. Objective. To assess coverage of ROP programs and their compliance with national guidelines. Study Design. Thirty-two neonatal intensive care units (NICUs) in five of the largest states were visited. Staff were interviewed to collect information on their ROP programs which were defined as (1) compliant, if National Guidelin… Show more

“…Historically, the regional incidence and severity of ROP have been measured in developed countries [18, 19]. In our study, the overall incidence of any stage of ROP was higher than the incidence for the second largest neonatal intensive care unit in Canada [20].…”

“…Antivascular endothelial growth factor (VEGF) agents have recently emerged as a therapeutic option. Bevacizumab (Avastin) is now used as a first-line treatment by up to a quarter of ophthalmologists following early studies suggesting superiority over laser in more central disease 40. However, the long-term neurodevelopmental and cardiovascular impact of the associated suppression of systemic VEGF levels, which can last for up to 8 weeks after intravitreous bevacizumab injection, is unclear.…”

“…Another limitation is the generalizability of the results outside the United States. Although variability in guideline adherence is not unique to the United States, 9,30 the burden of ROP disease is heavily weighted toward low-and middle-income countries 31 and often affects larger, more mature infants. Countries such as Mexico report difficulty with their ophthalmology coverage and access to equipment, leading to overall suboptimal compliance with their national guidelines.…”

“…Countries such as Mexico report difficulty with their ophthalmology coverage and access to equipment, leading to overall suboptimal compliance with their national guidelines. 30 The need for alternative solutions for ROP screening in developing countries was highlighted in 1 report investigating the use of a smart phone and a lens to provide remote screening. 32 A proposed "paradigm change" in the approach to ROP care has been made to address resource and personnel shortages, especially in low-and middle-income countries.…”
