Lipidsenker- und andere toxische Myopathien

Abstract: A growing number of therapeutic agents and exogenous toxins are harmful to structure and function of human skeletal muscle. The clinical syndrome encompasses asymptomatic creatine kinase elevation, myalgia, exercise intolerance, muscle paresis and atrophy, and lastly acute rhabdomyolysis. Toxic myopathies are potentially reversible, hence a prompt recognition is particularly helpful for the early diagnosis and in conclusion elimination of a myopathy inducing toxin. Toxic myopathies may be classified as acute o… Show more

“…Primary gonadal insufficiency may result from a direct toxic effect of ethanol and acetaldehyde on the gonads with inhibition of the enzymes responsible for the synthesis of sex hormones [39]. Alcohol excess induces acute and/or chronic neuropathy and myopathy [40,41]. Whether such neuropathy can affect erectile function remains to be established.…”

“…Among these, lipid reducers (statins) and colchicine in particular should be mentioned [ 70 ]. One risk which has been known for a very long time and which occurs with high frequency is the drug-induced myopathy elicited by corticosteroids (steroid-induced myopathy) [ 71 ].…”

“…Bisher sind über 4000 Fälle mit einer Lipidsenker-assoziierten Rhabdomyolyse mit 7 akuter Tetraparese, deutlicher CK-Erhöhung (>10fach der Norm), Myoglobinurie, Hyperkaliämie und Koagulopathie. Ein individueller Risiko-Haplotyp (die so genannte SLCO1B1-Variante) scheint hier enorm wichtig zu sein [15,19].…”
