Lipomatous Glioneurocytoma of the Posterior Fossa with Divergent Differentiation: Case Report

Abstract: A literature review is presented. Diagnostically, this neoplasm seems to fit in a unique group of rarely described, lipomatous neuroectodermal tumors that show divergent neuronal and glial differentiation.

“…Proliferation index, as measured by Ki-67/MIB-1, is usually low (<5%) in most published cases [1, 5, 6, 10, 17, 20, 23], which comforts the interpretation of the lesion as a benign one. However, many recent papers of separate cases reported relatively high rates of Ki-67 with cytopathologic atypia.…”

“…Bechtel et al named it “lipomatous medulloblastoma” and pointed to its better prognosis [7]. Since then, many other terms have been used to give name to this rare tumor: neurolipocytoma [8], medullocytoma [6], lipidized medulloblastoma [9], and lipomatous glioneurocytoma [10]. In 2000, the WHO adopted the term of “cerebellar liponeurocytoma” to replace all others.…”

“…On T1-weighted MRI the mass is generally hyperintense but heterogeneous [11]. Hyperintense "streaks" on T2-weighted images have been associated with the macroscopic appearance of adipose tissue at surgery [2]. Enhancement with gadolinium is usually heterogeneous with areas of tumour demonstrating variable degrees of enhancement.…”

“…Cerebellar liponeurocytoma is a rare tumor of the posterior fossa described at the first time by Bechtel et al in 1978 [1]; and since this date, it has been reported more than 42 times in the literature under many different names including lipomatous medulloblastoma, lipidized medulloblastoma, medullocytoma, and lipomatous glioneurocytoma [2]- [6]. Now, this tumor is recognized to be a distinct pathological entity in the revised WHO classification of tumors of the central nervous system, 2000, and considered like a rare WHO grade I or II well differentiated neurocytic tumor of the cerebellum with focal lipomatous differentiation [7].…”

“…Since the first report [1] by Bechtel in 1978 as a mixed mesenchymal and neuroectodermal tumor of the cerebellum, several terms "medullocytoma" [2], "lipomatous glioneurocytoma" [3] and "lipidized mature neuroectodermal tumor of the cerebellum" [4] have been proposed as to emphasize its resemblance to central neurocytoma.…”
