Lipomas are the most common benign mesenchymal tumors in adults. Generally, they are less than 5 cm in size but sometimes they can present as giant lipomas. Axillary region is an unusual localization for lipomas in general, especially for giant lipomas.A case of a 62-year-old male with a giant lipoma in the axillary region is presented. The tumor was located in the left axillary region beneath the pectoralis major muscle and extended all along to the scapula. It was adjacent to the axillary vessels and the brachial plexus, but did not invade the neurovascular bundle. A team of a plastic and thoracic surgeon performed the operation. The pathology report confirmed a benign lipoma weighting 454 grams and measuring 13.5x8x7 cm. Follow-up examinations (clinical examination and ultrasound) revealed no recurrence three years after the operation.The mechanism of lipoma formation is not completely clear. It includes two potential and overlapping mechanisms in which trauma seem to have the major role. The treatment includes surgical excision or liposuction. However, excision of large lipomas can sometimes be a surgical challenge.Giant lipomas in axillary region should be removed in order to establish the pathohistological diagnosis and to prevent possible compression of the neurovascular structures. In our opinion, surgical excision is a preferred method of treatment because there is less possibility of damaging vital structures and it offers better control against local tumor recurrence.

Alternative Cosmetic and Medical Applications of Injectable Deoxycholic Acid: A Systematic Review

BACKGROUND Beyond submental fat reduction, injectable deoxycholic acid (DCA) has gained popularity in recent years for various minimally invasive lipolysis applications. OBJECTIVE To summarize and evaluate the evidence of off-label uses of injectable DCA. METHODS MEDLINE, Embase, CINAHL, Web of Science, and CENTRAL were searched. The outcomes measured included applications of DCA, treatment regimen, and its efficacy. An overall success rate for each condition was calculated based on the improvement defined in the included studies. RESULTS Eleven studies evaluated the cosmetic use of DCA for excess adipose tissue on various anatomical locations. The outcomes were evaluated at time points ranging from 1 to 21 months post-treatment, with overall success rates over 85%. Eight case reports and series reported the success of using DCA treating lipomas, xanthelasmas, paradoxical adipose hyperplasia, fibrofatty residue of infantile hemangioma, piezogenic pedal papules, and HIV-associated lipohypertrophy. Although the preliminary efficacies were high, the overall recommendations for off-label uses are weak because of the lack of high-level studies. CONCLUSION The review emphasizes the diversity of injectable DCA as a minimally invasive technique for lipolysis. Further high-level studies demonstrating consistent treatment regimens and methods of evaluation are warranted to make more definitive recommendations regarding off-label DCA use.

Lipoma is the most frequent benign mesenchymal tumor. Giant intramuscular lipomas, defined by a measurement greater than 10 cm in any dimension, reside deep to the fascia and originate within the muscle layer. Traditionally, the length of the incision required for excision is equal to or greater than the diameter of the giant intramuscular lipoma. However, because of injury to the surrounding tissues from excessive traction and thermal injury from the energy delivered by the device, long and noticeable scars with a hypertrophic tendency develop, resulting in patient dissatisfaction. Although some esthetically-favorable methods have been reported, including liposuction and liposuction-assisted excision, these methods did not sufficiently excise lipomas, and led to a higher recurrence rate. In contrast, the ultrasonic harmonic scalpel can excise large and deep-seated lipomas with minimal scarring. The ultrasonic harmonic scalpel requires a shorter incision, and it provides a sufficient view of the surgical field, thereby avoiding unnecessary traction applied to the skin and reducing surrounding tissue injury trauma, which results in hypertrophic scars.
