Live/Real Time Three‐Dimensional Transthoracic Echocardiographic Description of Chordoma Metastatic to the Heart

Abstract: We describe live/real time three-dimensional transthoracic echocardiographic features of a chordoma metastatic to the heart and correlate its appearance with the surgical pathology.

“…It also allows the manipulation of images in the space, obtaining views and planes, and aligning structures in ways that were impossible to get before with 2D imaging. Emphasis can then be put on the cardiac mass, providing a more comprehensive assessment of its inner structure, which correlates better with pathologic fi ndings (necrosis, hemorrhage, cystic areas, or fi brotic bands) [4,5].…”

“…RT3DTTE is particularly valuable for evaluation of cardiac carcinoid because (1) classically the tricuspid valve is involved, and (2) RT3DTTE is particularly noted for having strength for imaging the tricuspid valve enface and therefore visualizing all three leaflets. 34,35 Cardiomyopathy/Noncompaction: Noncompaction of the LV can lead to a nonischemic cardiomyopathy and the sinusoids (noncompacted muscle) in the apex are mass-like and reviewed briefly here. Chordoma rarely metastasizes to the heart.…”

“…Echodense areas by 3D correlated with fibrotic areas of the tumor by pathology and similarly, echolucencies correlated with areas of necrosis. 34,35 Cardiomyopathy/Noncompaction: Noncompaction of the LV can lead to a nonischemic cardiomyopathy and the sinusoids (noncompacted muscle) in the apex are mass-like and reviewed briefly here. In a case series and an additional case, RT3DTTE showed incremental value over 2D echocardiography by diagnosing noncompaction in the RV.…”

“…The present study demonstrates that 3D TTE has incremental value in the assessment of RV tumors. One of the inherent properties of 3D TTE is its ability to sequentially section the tumor and view it from multiple angles, giving the physician a look at the inner part of the tumor 6 . Examining the inner structure of the tumor by 3D TTE, and noting the characteristics of the mass, has been shown to correlate well with the findings of pathologic analysis of the tumor 6 .…”

“…The study included 11 patients (2 males, 9 females, age range 19–64 years, mean age 33.9 years) who were first evaluated by a standard 2D TTE, and this was followed by live/real time 3D TTE. Two of the 11 patients have been previously reported 5,6 (Table I). All study subjects underwent echocardiographic examination with a Phillips iE33 ultrasound imaging system (Andover, MA, USA), as previously described 3,7 …”
