Liver hydatid cyst with cystobiliary communication: Laparoscopic surgery remains an effective option

Abstract: Introduction:Most centres offer laparoscopic treatment for liver hydatid cyst (LHC). There have been concerns about the management of intra-peritoneal spillage, bleeding, and cystobiliary communication (CBC) during laparoscopic surgery for LHC. CBC can exist in 13%–37% of cases of LHC. No randomised studies have compared open versus laparoscopic approach for the treatment of LHC. We specifically analysed the outcomes of laparoscopic treatment of LHC with special reference to associated biliary complications.Pa… Show more

“…The biliocystic communication was closed by either clipping or placing a laparoscopic suture, depending on the size and the surgeon’s experience, although this method can sometimes prove difficult to be achieved laparoscopically, thus supporting the observed statistically significant difference between the laparoscopic and open approaches regarding the presence of cystobiliary fistula. However, due to the improved imaging and the possibility of postoperative ERCP for lowering the biliary pressure or in case of the presence of biliary fistulas, the laparoscopic approach is more widely considered, even in the presence of biliocystic communication, with more studies supporting this statement[ 11 - 13 ].…”

“…Bile leakage is recognized as the most common complication following surgical management of hydatid liver cysts, with reported incidences ranging from 6.2 to 36.2% in the literature [ 15 18 ]. In our series, the incidence of bile leakage was 14%, which is reasonable considering the inclusion of cysts with biliary communication.…”
