Liver stiffness and serum markers for excluding high-risk varices in patients who do not meet Baveno VI criteria

Abstract: BACKGROUNDThe Baveno VI criteria for predicting esophageal varices, i.e., liver stiffness measurement (LSM) < 20 kPa and platelet (PLT) count > 150 × 109/L, identify patients who can safely avoid gastroscopy screening. “…A positive correlation between the proportion of NAFLD and rate of spared endoscopy was reported [ 29 ]. In a previous study, we found that, among patients with HBV-related compensated liver cirrhosis who did not meet the Baveno VI criteria, the prevalence of HRV was significantly lower in those with ALT or total bilirubin (TBil) ≥ 2 upper limit of normal (ULN) (14.3%) than in those with both ALT and TBil < 2 ULN (34.1%) [ 40 ]. These findings suggested that the concomitant liver inflammation may falsely increase LSM, making it difficult for patients to fulfill the Baveno VI criteria and decreasing the rate of spared gastroscopies.…”
