Liver Transplantation for Treatment of Giant Hepatocellular Adenomas

Abstract: Giant hepatocellular adenomas are associated with a high incidence of rupture with intraabdominal hemorrhage and may also undergo malignant transformation. If resection is not technically feasible, liver transplantation should be a treatment option. The aim of this report is to describe the indications, feasibility, and outcome of liver transplantation for hepatocellular adenomas. A 66‐year‐old man with a 17‐cm hepatocellular adenoma originating in the left lobe but involving nearly the entire liver and a 35‐y… Show more

“…If patient presents with multiple hepatic adenoma, complete resection may be impossible, and hepatic transplantation is taken into consideration when tumors progress. 21 Malignant transformations is rare, but there have been a few cases reported. 22 In one series of 39 cases of unresected liver adenoma, 5 cases subsequently developed hepatocellular carcinoma.…”

“…The risk of malignant transformation to hepatocellular carcinoma is 5-10% [1,5]. The natural history of tumor is uncertain, and thus the spectrum of treatment of hepatic adenomas range from careful observation with cessation of oral contraceptives to liver resection to orthotopic liver transplantation in unresectable cases [5,8].…”

“…Surgical treatments such as lobectomy [21,33] , enucleation [34] and liver transplantation [35] are perfor med for LCA. Percutaneous ethanol injection [20] and transcatheter arterial embolization [33] are established therapies for low invasive treatment of HCC, and were reported to be performed for LCA.…”
