Localization of Lymphatic Leakage Site in Chylothorax by Thoracic Duct Scintigraphy by Orally Administered 123I BMIPP Using SPECT/CT

Abstract: A 58-year-old man with chylothorax after graft replacement of the aortic arch underwent thoracic duct scintigraphy by orally administered 123 I-labeled 15-(4-iodophenyl)-3(R,S)-methylpentadecanoic acid with single-photon emission computed tomography combined with computed tomography (SPECT/CT). SPECT/CT fusion image clearly showed the increased uptake near the vascular anastomotic site of the aortic arch. Therefore, a chylothorax due to a traumatic rupture of a thoracic duct was suspected. “…This approach has been supported by numerous authors and has yielded satisfactory results in terms of reducing chyle production within a few days to 3-4 weeks. [14] In some cases, administration of octreotide has shown positive outcomes for patients. [15] Surgical intervention Chest Tube Placement and Pleurodesis Regimen the patient underwent a chest tube thoracostomy procedure along with bed rest, total parenteral nutrition, and maintenance of electrolyte balance.…”

“…The presence of chylomicrons could be also speculated with the measurement of fluid cholesterol and triglyceride levels [12]. In recently published data near-infrared fluorescence imaging of thoracic duct and scintigraphy by orally administered 123 I BMIPP using SPECT/CT could reveal the chylous leakage [37]. Treatment of traumatic chylothorax is mainly conservative with chest drainage and total parenteral nutrition (TPN), followed by medium-chain triglyceride diet.…”
