Long Survival Following Bacterial Meningitis-Associated Brain Destruction

Abstract: This report describes the brain autopsy of a boy who at age 4(1/2) years experienced an episode of fulminant Haemophilus influenzae type b bacterial meningitis, resulting in massive brain destruction and the clinical signs of brain death. However, medical intervention maintained him for an additional two decades. Subsequent autopsy revealed a calcified intracranial spherical structure weighing 750 g and consisting of a calcified shell containing grumous material and cystic spaces with no recognizable neural el… Show more

“…15,16 There were a total of 1012 reported sequelae, of which a majority were behavioral and/or intellectual deficits (n ϭ 792, 78.3%). Specific diagnoses and conditions include low intelligence quotient (IQ)/ cognitive impairment (n ϭ 455, 45.0%), attention deficit hyperactivity disorder (n ϭ 24, 2.4%), and behavioral deficits (n ϭ 77, 7.6%).…”

“…Given the cited cases-the oldest patient (T.K.) being twenty-four years old and pre-pubescent when he suffered whole-brain death (Repertinger et al 2006)-it appears that the organic systems of young children, possibly up to some point around the onset of puberty, are more "plastic" than those of more mature human beings. 25 Perhaps the integrative functions normally carried out by the brainstem in mature human beings can be taken on by other, normally less critical, neural structures in young children or even adolescents.…”

“…11 The bodies of such patients manifest many holistic properties, such as homeostasis, proportional growth, and overcoming intercurrent illnesses. These factors include direct damage to vital organs by the initial etiology, secondary damage to those organs from sympathetic storm during brain herniation, a widespread inflammatory reaction to tissue damage, and autonomic dysfunction from spinal shock.…”
