Long-term follow-up of bucket-handle meniscal repairs: chondroprotective effect outweighs high failure risk

Abstract: PurposeOutcomes after repair of bucket‐handle meniscal tears tend to be satisfying in the short‐term follow‐up. However, the literature is scarce regarding long‐term data following repair of bucket‐handle meniscal tears. The aim of this study was to assess long‐term follow‐up outcomes, focusing on knee osteoarthritis (OA) development and failure rate, and determine risk factors associated with failure. MethodsThis is a retrospective cohort study, including all patients with bucket‐handle tears within 4 mm of t… Show more

“…Beginning in the 1970s, it was recognised that the full integrity, or at least a maximal part, of menisci could be saved by repairing the ruptured areas in the red–red and red–white zones, thereby preventing early arthrosis. Therefore, repair, rather than primary resection of ruptured menisci is, whenever possible, currently considered the treatment of choice [1–3, 7, 8, 11, 12, 14, 15, 21, 22, 28, 30–33].…”

“…The present study, therefore, compared outcomes in patients who underwent meniscus repair within 3 weeks and after more than 3 weeks after the trauma, the hypothesis being that successful repair of meniscus tears within 3 weeks would result in fewer failures, which result in (partial) resection of the ruptured part of the meniscus after all. Early arthrosis could so be prevented in those knees in which the repair was successful [1–3, 7, 8, 11, 12, 14, 15, 21, 22, 28, 30–33]. All operations were performed by one surgeon using the same technique.…”

“…It is critical to accurately diagnose a displaced BHMT based on MRI and clinical presentation because surgical intervention is necessary to restore and preserve meniscal function; delay meniscal degeneration, fragmentation, or permanent deformation; and prevent potential long-term osteoarthritic changes 25 . The gold standard treatment is arthroscopic repair using an inside-out suture technique 26 .…”

“…The importance of preserving the meniscus, especially for preventing degenerative changes, has been documented [1,2]. Meniscal repair is considered the treatment of choice for medial meniscus injuries whenever possible [3,4].…”
