Long-term follow-up of DDH patients who underwent open reduction without a postoperative cast

Abstract: We present the results - assessed after bony maturation - of an early anterior approach open reduction performed using our modified technique on 49 hips at 6-24 months of age. We start postoperative functional treatment using a Pavlik harness and an abduction splint, abandoning plaster cast. Secondary surgeries were performed in 11 hips (22.4%) at 3-7 years of age. Our results were 'satisfactory' (Severin Grade I, II) in 96% of the cases. According to the joints' Severin Grade, the total hip replacement probab… Show more

“…The radiographic outcome of open reduction in Group B was good with Severin Grade I/II hips in 93%. Our results are in accordance with previous studies of open reduction using the anterior approach with Severin Grade I/II in 96% of the hips at skeletal maturity [26] and in 92% at a mean followup of 6 years [2]. However, considerably worse results after open reduction were reported by Holman et al [7] with only 33% Severin I/II hips after a mean followup of almost 30 years.…”

“…We found no significant difference in the proportions of AVN between the two treatment groups. Our rate of AVN Groups II to IV was within the range of 5% to 43% in previous studies on open reduction [1,7,14,26]. AVN occurred more frequently after open reduction than after closed reduction in Group B, but most were Group II and had a good prognosis.…”

“…In terms of radiological results, open reduction through an anterior approach is reported to be successful in many studies. In their long-term study, Szepesi et al (36) performed open reduction through an anterior approach in 49 hips of patients aged 6-24 months. They reported radiologically satisfactory results in 96% according to the Severin criteria.…”

“…It is known that the final result of DDH after open reduction can be evaluated only when musculoskeletal growth is completed [81], but to our knowledge, there is a paucity of studies including skeletally mature patients [5,18,30,33,39,52,71,[78][79][80].…”

“…During the last 36 years, the anterior approach has been reported less frequently [1,10,17,18,20,23,25,30,31,47,58,70,71,73,79,80] than the medial approach [4, 6, 7, 11, 16, 21, 22, 33, 36, 37, 39, 40, 46, 48-50, 52-54, 60, 69, 77, 78, 85, 88], especially in younger children.…”
