Long-Term Neurological Outcomes in West Nile Virus–Infected Patients: An Observational Study

Abstract: The Houston West Nile Cohort (HWNC) was founded in 2002 when West Nile virus (WNV) reached Houston, TX. The long-term outcomes following WNV infection are still mostly unknown, though neurological abnormalities up to 1 year postinfection have been documented. We report an observational study of neurological abnormalities at 1–3 and 8–11 years following WNV infection in the HWNC. We conducted standard neurological examinations at two separate time points to assess changes in neurological status over time. The m… Show more

“…4; Additional file 1: Table S3). Most studies examined acute mortality ( n  = 41), while four studies examined long-term mortality (two to 10 years) [23, 4851]. Five studies differentiated deaths attributable to WNV [14, 52–55], while the remaining examined all-cause mortality.…”

“…Most presented case fatalities calculated as the number of deaths as a proportion of the number of WNV cases. Few presented survival models, namely Kaplan-Meier curves [23, 4851].

“…As accurately stated by Majde [104], long-term neurodegenerative disorders may represent a "hit-and-run" type of pathology, since some symptoms are triggered by innate immunity associated with glial cell activation. Different forms of long-term sequelae (cognitive deficits and behavior changes, decreased memory/learning, hearing loss, neuromuscular outcomes/muscular weakness) were also observed following arboviral infections [83,103,[105][106][107].…”

“…55 This shows that survival is not always unaccompanied by consequences. 56 Moreover, it appears that transmission can occur between mosquitoes during multiple bites, lessening the possibility of viral extinction. 57 Military strategy teaches us to attack weak points of an enemy, not the strongly defended ones.…”
