Long-term outcome after allogeneic hematopoietic stem cell transplantation for advanced stage acute myeloblastic leukemia: a retrospective study of 379 patients reported to the Société Française de Greffe de Moelle (SFGM)

Abstract: Summary:To assess the place of allogeneic hematopoietic stem cell transplantation (HSCT) in the advanced stage of acute myeloid leukemia (AML), we retrospectively analyzed 379 consecutive patients who underwent allogeneic HSCT for advanced AML. The median follow-up of the entire cohort was 7.5 years. Sixty-nine patients (18%) were transplanted with primary resistant disease. Three hundred and ten (82%) were relapsed patients, 94 (30%) of whom were in untreated relapse, 67 (22%) in refractory relapse and 149 (4… Show more

“…9 It is clear that patients with active leukemia had more relapse and worse OS after allo-HCT regardless of donor type or patient age. [10][11][12][13][14][15] Following allo-HCT, 40-60% of AML patients in CR1 enjoy long-term OS, whereas o20% of refractory or relapsed AML patients survive. 1,11 Yet even for allo-HCT during CR, relapse remains the most frequent complication of allo-HCT.…”

“…12,32,[39][40][41] The presence of active disease at the time of transplantation remained the strongest predictor of inferior transplant outcome, indicating that this cohort of patients should be considered for alternative therapeutic strategies. The role of dose-escalated conditioning regimens or sequential chemotherapy followed by RIC HSCT has been investigated by other groups in this context, with promising results.…”

“…This corresponds to published results, demonstrating an increased NRM in advanced or uncontrolled disease. 24 The concept of a delayed GvL reaction seemed to work as well, as shown by the low incidence of both severe aGvHD and early relapse, and by the fact that eight patients (50% of those achieving CR after HSCT) were able to receive pDLT. Under the conditions applied, pDLT was safe, even in the unrelated setting, causing mild GvHD in three out of eight patients.…”
