Long-term outcome of patients with mild intermittent claudication under secondary prevention

Abstract: A high vascular comorbidity also develops under secondary prevention as an expression of the continuing generalization of the atherosclerotic process in PAD. The causes for death are determined both by the vascular and the tumor related comorbidity.

“…A total of 74 major vascular events and 111 revascularizations were recorded in the follow-up. 11 However, both vascular mortality and total mortality in the 10-year period were well below those described for the natural history and also in studies on secondary prevention measures in claudicants. [17][18][19][20][21] This needs to be discussed in the context of the inclusion criteria chosen for the study: none of the study patients had a vascular or serious nonvascular comorbidity at the start of the study and renal failure was an exclusion criterion.…”

“…Patient recruitment details have been previously described. 11 Secondary prevention measures were initiated or continued in all patients. Secondary prevention included therapy for all treatable vascular risk factors and antiplatelet therapy.…”

“…Despite the growing body of literature identifying the polyvascular phenotype as a malignant condition with extremely high cardiovascular risk, little is known about the progression of patients with PAD alone toward a polyvascular disease. Results from a prospective observational study with 109 claudicants (Fontaine stage IIa) showed that an aggravation towards a polyvascular disease occurred in over 60%, with 20% in all three vascular beds [32]. Mortality was similarly high in our population at 26%, including 39% deaths from vascular causes.…”
