Long-term remission and recurrence rates after first and second transsphenoidal surgery for Cushing's disease: care reality in the Munich Metropolitan Region

Abstract: Objective: Transsphenoidal surgery (TSS) presents the treatment of choice for Cushing's disease (CD). Remission and recurrence rates vary dependent on tumor size, extension, adenoma visibility on magnetic resonance imaging, and neurosurgical expertise. Other than published from single-surgeon neurosurgical series so far, we have aimed to describe long-term remission and recurrence rates of CD in a series incorporating different neurosurgeons, trying to reflect care reality in the Munich Metropolitan Region, wh… Show more

“…In patients with Cushing disease (CD), the most common form of endogenous CS, surgical success rates are variable, ranging from 65 to 90% when performed by an experienced surgeon (6,7). Among those patients with successful outcomes following transsphenoidal surgery, 11 to 66% will go on to develop recurrence of hypercortisolism with long-term follow-up (6,(8)(9)(10)(11)(12). Therefore, additional medical interventions are often required to achieve remission or improve the clinical manifestations of CS.…”

“…In total, 36 patients underwent repeat transsphenoidal surgery, and remission was achieved in 42%. Second recurrence occurred in 40% of patients after a mean of 27 months (14). In our institution, we consider second transsphenoidal surgery in all cases of persistent or recurrent CD following a careful review by our multidisciplinary pituitary tumour board.…”

“…Corticotroph adenomas are typically isolated tumors consisting of a monoclonal cell population in a pituitary of normal size (5,6,7). Selective transsphenoidal adenomectomy causes remission in 60-96% of patients (8,9).…”
