Long-Term Results (≤18 Years) of the Edge-to-Edge Mitral Valve Repair Without Annuloplasty in Degenerative Mitral Regurgitation

Abstract: Preoperative, intraoperative, and postoperative data were collected through a hospital database and patients' records. Follow-up data were Background-To assess the long-term results of the edge-to-edge mitral repair performed without annuloplasty in degenerative mitral regurgitation (MR). Methods and Results-From 1993 to 2002, 61 patients with degenerative MR were treated with an isolated edge-to-edge suture without any annuloplasty. Annuloplasty was omitted in 36 patients because of heavy annular calcificatio… Show more

“…When MR is eliminated by surgical MV leaflet repair, it is controversial whether ring implantation is still necessary. 22 This study may provide an insight for ring implantation even after the elimination of MR with surgical leaflet repair. In addition, MRI study has demonstrated frequent myocardial fibrosis in the LV base in patients with MVP.…”

“…Indeed, in the surgical experience, a suboptimal repair with residual prolapse and 2þ MR is a well-known predictor of recurrent severe MR. 15 MitraClip repair is a leaflet procedure and lacks annuloplasty, which is fundamental to achieve optimal and durable results in surgery. We recently published our long-term results with the edge-to-edge technique without annuloplasty in the surgical setting, 16 reporting a disappointing freedom from MR recurrence of 64% at 10 years despite an initial optimal result (MR 1). Transcatheter mitral annuloplasty devices are emerging with promising perspectives, and we all look forward to combine the procedures to recreate in the near future a complete surgical percutaneous mitral repair.…”

“…In degenerative MR, the overall long-term results of the surgical edgeto-edge technique without annuloplasty were found to be unsatisfactory by Alfieri and colleagues. 25 Despite the analogy, percutaneous MitraClip therapy has different mechanical effects compared with the surgical Alfieri technique. Percutaneous clipping exerts greater forces on the MV apparatus as compared with the operative technique leading to a decrease in annular area and diameter comparable with surgical or percutaneous annuloplasty.…”
