Long-Term Results after Metatarsal Head Resection in the Treatment of Rheumatoid Arthritis

Abstract: Our results, which are comparable to those of other studies, confirm the success of metatarsal head resection for the treatment of inflammatory forefoot destruction in rheumatoid arthritis to correct deformity, reduce pain, improve ambulation, and offer the patient a greater variety of shoewear.

“…Other studies have listed wound healing problems in the range of 4% to 24% and infection rates in the range of 0% to 14%. [8][9][10][11][12][13] If a deep infection occurs postoperatively and there is a suspicion of bone infection, hardware should be removed as soon as possible and a bone biopsy should be performed to definitively determine the presence of osteomyelitis. If bone infection is indeed present, the use of external fixation may prove beneficial to maintain bony alignment while antibiotic spacers and intravenous antibiotics are used to treat the infection.…”

“…Although joint-preserving procedures have been regarded as standard treatment for chronic metatarsophalangeal dislocation of deformed lesser toes, resection arthroplasty of metatarsal head is a reasonable alternative approach for patients with rheumatoid arthritis. 21 Resection arthroplasty through plantar approach offered good results, including reducing pain and improving ambulation, in a 20-year long-term follow-up study in which more than 80% of patients felt satisfied with the outcome. 23 Despite good results reported with resection arthroplasty, some surgeons may still prefer to use modified metatarsal shortening offset osteotomy for joint preservation even in rheumatoid cases.…”
