Long-term Results of Olfaction Rehabilitation Using the Nasal Airflow–Inducing ("Polite Yawning") Maneuver After Total Laryngectomy

Abstract: The effectiveness of the NAIM, or so-called polite yawning technique, for the rehabilitation of olfaction in individuals who have undergone total laryngectomy was reconfirmed. Long-term olfaction rehabilitation was achieved in about 50% of the patients, but more intensified training may be needed to increase the percentage of successfully rehabilitated individuals. The SDT is an effective and simple test for the assessment of olfaction acuity after laryngectomy.

“…The mean post-intervention score reached 29.24, indicating moderate microsmia. Such result corroborates those of previous studies (2,5,10,15,26,27) . A change in the olfactory perception of a considerable number of patients was observed, with an increased percentage of individuals able to perceive some smells.…”

“…Nevertheless, a long-term assessment showed that not all individuals who learn the maneuver are able to execute it properly (27) . Thus surveys subsequent to the aforementioned study used more intensive rehabilitation programs with a larger number of sessions (5,23,26) to ensure the maintenance of long-term results.…”

“…This lead in 2000 to discovery of the Nasal Airflow Inducing Manoeuvre (NAIM), also called 'Polite Yawning' manoeuvre, since the latter describes the feeling well: enlarging the oral cavity by lowering the jaw and keeping the lips closed and the soft palate relaxed. 11,12 In subsequent intervention studies, the effectiveness of this method for olfaction rehabilitation was proven. Since then, studies in Sweden and Australia have confirmed efficacy of the NAIM and thus have given this rehabilitation method 'level I evidence status'.…”

“…11,[13][14][15] Despite the efficiency of this technique in improving patient quality of life after laryngectomy, the effect of different nasal variables on the outcome of this procedure and on outcome stability are unclear. 16 However, nasal endoscopy findings such as non-use rhinitis can be considered probable effective factors. 1,6 Changes in appetite and taste perception after rehabilitation manoeuvres were also considered.…”
