Long-Term Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) Infectiousness Among Three Immunocompromised Patients: From Prolonged Viral Shedding to SARS-CoV-2 Superinfection

Abstract: Guidelines for stopping COVID-19 patient isolation are mainly symptom-based, with isolation for 10 to 20 days depending on their condition. Here, we describe three deeply immunocompromised patients, each with different clinical evolutions. Asymptomatic carriage, symptom resolution, or superinfection with a second SARS-CoV-2 strain were observed, all leading to prolonged infectious viral shedding several months. We followed the patients epidemiological, clinical, serological data, infectiousness using viral cul… Show more

“…There have been relatively few published descriptions of persistent SARS-CoV-2 infection, the majority associated with B cell immunodeficiency (7,10,27,(41)(42)(43)(44). Conversion to PCR negativity and dramatic symptomatic improvement was achieved only after administration of casirivimab and imdevimab.…”

“…The mutations that do not follow this trend are L452R, F490S, T478K, and S494P; we thus speculate that their benefits for viral fitness are likely connected with the host immune system. The immunocompromised patients, treated or non-treated with convalescent plasma or neutralizing antibodies, have been regarded crystal balls for prediction of viral evolution (7,8), showing the emergence of N501Y, S494P, Q493K, Y489H, E484K, T478K and N440D (9, 10) variants during infection. Surprisingly, only mutations E484K and N501Y arose convergently among patients, and their emergence was restricted to a low number of patients.…”

“…. The intra-host SARS-CoV-2 genomic diversity (6) and the viral evolution in immunocompromised patients (7)(8)(9)(10) also received a lot of attention. Yet studies focused on convergent (syn.…”

“…Thus, the patient was probably exposed to different nosocomial circulating strains that might have caused E1 superinfection. To the best of our knowledge, only another likely SARS-CoV-2 superinfection has been reported (Tarhini, 2021), in an immunosuppressed patient, for whom two SARS-CoV-2 strains were identified on Day 56. There is higher risk of prolonged viral shedding in immunosuppressed SARS-CoV-2 positive cases (Choi et al, 2020), which may explain the superinfection.…”
