Long-term survival after chemoembolization of metastatic right atrial tumor thrombus as a presenting feature of hepatocellular carcinoma: A case study

Abstract: Abstract. Advanced hepatocellular carcinoma (HCC) with invasion into the heart through the hepatic vein is a recognized rare occurrence with an extremely poor prognosis. Patients who present with right heart tumor thrombus have generally been considered inoperable. Although aggressive resection and liver transplantation treatment have previously been performed, the results remain unsatisfactory. However, HCC with extension into the heart usually indicates a contraindication for transcatheter arterial chemoembo… Show more

“…Surgical thrombectomy is the main therapeutically option but, radiofrequency ablation, chemoembolization, thalidomide, and sorafenib have also been described as feasible alternatives for advanced hepatocellular carcinoma, despite the high mortality rate within a couple of months whether the patient is treated or not 19 , 26 , 28…”

“…Such TTs are usually found in imaging studies. Complete obstruction of the HV and IVC may cause Budd-Chiari syndrome ( 10 ). The patient can present with manifestations such as varicose veins of the esophagus, fundus, upper extremity in relation to varicose veins of bilateral upper limbs, thoracic cavity and abdomen, pleural effusion, lower extremity edema, tachycardia and difficulty breathing, syncope, repeated pneumonia and sudden death ( 6 , 11 ).…”

“…Complete/partial regression of liver and RA tumor, α-fetoprotein decrease, and symptom-free were observed at 1-4 months [1]. Selective embolization with pirarubicin 30 mg, oxaliplatin 200 mg, hydroxycamptothecine 20 mg, and iodized-oil was also reported, obtaining similar effects to thalidomide [46].…”
