Long-term Survival After Surgery Compared With Concurrent Chemoradiation for Node-negative Small Cell Lung Cancer

Abstract: In a national analysis, surgery with adjuvant chemotherapy was used in the minority of patients for early stage SCLC. Surgery with adjuvant chemotherapy for node-negative SCLC was associated with improved long-term survival when compared to concurrent chemoradiation. These results suggest a significant underuse of surgery among patients with early stage SCLC and support an increased role of surgery in multimodality therapy for cT1-2N0M0 SCLC.

“…In Quoix et al (17) study published in 1990, they found a pulmonary nodule in 25 of 408 SCLC patients during a 5-year period. Additionally, a total of 2301 patients with T1 and T2 N0 small cell lung cancer in the national cancer database between the years of 2003-2011 were reviewed in Yang et al (18) latest article published in 2017. Surgical treatment and chemotherapy were used together in 681 of these patients who had a solitary pulmonary nodule.…”

“…Two large cohort study suggests an increased role of surgery in multimodality therapy for early-stage SCLC. 3,4 These results might indicate that surgery could be a treatment option even in locally-advanced disease after appropriate patient selection.…”

“…10,11 However, in SCLC, NLR and PLR studies were reported exclusively from East Asia, and data have not yet been published on Caucasian or resected stage populations. [1][2][3][4][5][6] All in all, although surgical treatment of SCLC is challenging, in this study, we aim to identify biomarkers that help in patient selection that potentially benefit from lung resection.…”

“…While there are no recent randomized controlled trials (RCTs) of acceptable quality evaluating the use of surgery for early stage SCLC (7), large database studies have suggested that there may be a survival benefit to surgical resection (8)(9)(10)(11)(12). These findings have supported the current inclusion of surgical therapy in the National Cancer Care Network (NCCN) guidelines for early stage SCLC.…”
