Long‐Term Survival in Patients Treated with Cardiac Resynchronization Therapy: A 3‐Year Follow‐Up Study from the InSync/InSync ICD Italian Registry

Abstract: During long-term (3 years) follow-up after CRT, total mortality rate was 10%/year. The outcome of ischemic patients was worse mainly due to a higher rate of death from progressive heart failure. Ischemic etiology along with NYHA class IV was identified as predictors of death. Benefits of CRT in terms of clinical function and echocardiographic parameters persisted at the time of long-term follow-up.

“…Finally, there were no significant differences in CRT-related complications in both patient groups. Of note, serious complications such as cardiac tamponade (reported in 0.5-1% in COMPANION 4 and the Italian InSync Registry 18 ), infection and explantation (1.3% in MIRACLE, 2 1% in CARE-HF 3 ), or peri-procedural mortality (0.8% in COMPANION 4 ) were not observed in our patients. A dissection between the wall of the vena cava superior and ingrown old ICD leads occurred in a patient undergoing CRT upgrade but remained without clinical sequelae, and did not prevent continuation of the procedure and successful CRT implantation.…”

“…13 From the other two studies, one did not include a control group, and was therefore purely descriptive in nature. 18 Marai et al compared 25 patients with upgrade procedure with 73 patients with de novo CRT implantations. 16 They reported similar changes in echocardiographic parameters, NYHA functional status, and 6 min walking test.…”

“…1 About 20% of patients treated with cardiac resynchronization therapy (CRT) are in AF. 2,3 Despite the high prevalence of AF in patients with HF and the fact that many meet the criteria for CRT, randomized controlled trials have excluded these patients in most cases. 4 Thus, the effect of this therapy in patients with AF is still controversial.…”

“…In Europe, 24% of CRT recipients are women [78], and the corresponding figure in the USA is 22-26% [73], with women comprising 17-24% [79][80][81][82] in larger registry studies of CRT. Although CRT appears to be underutilized in women, the reason for this is unclear.…”
