Longitudinal trends in PrEP familiarity, attitudes, use and discontinuation among a national probability sample of gay and bisexual men, 2016–2018

Abstract: This study explored familiarity with, attitudes toward, uptake and discontinuation of PrEP (Pre-exposure prophylaxis) among a national probability sample of gay and bisexual men. PrEP is one of the most effective biomedical HIV prevention strategies; however, use among gay and bisexual men remains low within the United States. This study used a national probability sample of gay and bisexual men from three age cohorts of men (18–25, 34–41, and 52–59 years at wave 1) who completed three annual surveys between M… Show more

“…Third, previous estimates represent all PrEP-eligible individuals and assume adherence and persistence are constant across risk groups (eg, people who inject drugs, high-risk heterosexuals, and MSM). In reality, there may be important differences by risk group or age, and ignoring this heterogeneity might result in biased estimates of PrEP impact [ 47 , 48 ]. By focusing on MSM, who have the highest burden of HIV infection of all risk groups and the most unmet need for PrEP [ 9 ], our method will produce estimates that are directly informative to a trial counterfactual for MSM and to evaluating prevention needs of this important population.…”

“…Advancements in cost coverage over the past decade (eg, Medicaid expansion, federal and drug company medication assistance programs, and federal clarification that insurers must pay for all PrEP-related costs) are helping to ease financial concerns among MSM, [18][19][20] however not universally for states that have not expanded Medicare. Stigma of using PrEP was also not a common primary reason for men refusing PrEP 21 as it was earlier in the era of PrEP implementation, as social norms adjust when more MSM use PrEP. 22,23 Among the top reasons for refusing daily oral PrEP recommended by a provider were potential side effects.…”

“…Oral pre-exposure prophylaxis (PrEP) is a highly effective antiretroviral regimen against HIV infection when taken as prescribed (Chou et al, 2019; Grant et al, 2010; Spinner et al, 2016). With increasing PrEP awareness and acceptability among MSM (Holloway et al, 2020; Sullivan et al, 2020), routine PrEP discussions with health care providers are essential. Without such discussions, providers may miss opportunities to screen for PrEP or offer PrEP to their patients, including MSM who experience sexual violence.…”
