Longitudinally extensive transverse myelitis with seropositive chikungunya

Abstract: Chikungunya viral (CHIKV) fever is often a self-limiting febrile illness associated with severe debilitating arthralgia. Neurological complications associated with CHIKV, although rare, have been reported in literature; however, longitudinally extensive transverse myelitis (LTEM) is rarely associated with it. We present a case of a middle-aged man with a 1-week history of low-grade fever and arthralgia followed by urinary retention and quadriplegia. A sensory level was noted at T2. On subsequent investigations… Show more

“…Although the by which SARS-CoV-2 causes ATM (Acute Transverse Myelitis) is not yet entirely clear, an increasing number of studies have linked ATM as a neurological complication of COVID-19 [5][6] . The possible pathways through which the SARS-CoV-2 virus affects the spinal cord and causes ATM (Acute Transverse Myelitis) include direct invasion of the spinal cord, cytokine storm, and autoimmune response.…”

“…6 Myelitis related to CHIKV infection is seldom reported. 3,[7][8][9][10] Myelopathy as an isolated neurological presentation occurs infrequently (20%). 4,10 A few prior reports of diffuse, T2 hyperintense lesions involving the cord in the entirety with no significant postcontrast enhancement exist.…”
