Amiodarone Low back pain: case reportA 39-year-old man developed debilitating low back pain following administration of intravenous amiodarone.The man with atrial fibrillation was diagnosed with occlusion of left anterior descending vessel and underwent coronary artery bypass graft surgery. He received IV amiodarone 150 mg over 10 minutes as a bolus followed by an infusion of 1 mg/min for 2 hours and 0.5 mg/min for 13 hours for the treatment of atrial fibrillation on the day of surgery. Starting on the post operative day (POD) 1, he was transitioned to oral amiodarone 400mg twice daily for 2 days and then 200mg twice daily for 2 days. However, he continued to experience intermittent atrial fibrillation which required a bolus dose of IV amiodarone 150 mg over 10 minutes on POD 5 and again on POD 6. His oral amiodarone dosage was also increased to 400 mg twice daily. On POD 6, 5 minutes after his bolus infusion, he developed debilitating low back pain that radiated to his chest and arms with concurrent hypertension and anxiety.The man's amiodarone bolus was stopped and within 1 minute his pain resolved. His oral amiodarone was continued and he was discharged the next day on decreasing doses of amiodarone.
