Low seroprevalence of Helicobacter pylori infection in patients with stress ulcer bleeding

Abstract: Our results suggest that the Helicobacter pylori infection does not play a pathogenic role in stress ulcer bleeding. Prophylactic cure of Helicobacter pylori can not be recommended in this setting.

“…In fact, it has been well-documented that physical stress encountered in the intensive care unit can induce peptic ulcer bleeding among Hp -negative patients [8-10]. On the other hand, our two patients developed hemorrhagic gastric ulcers due to psychological stress rather than to physical pathologies.…”

“…Severe stress of life-threatening conditions, eg shock, head trauma, myocardial infarction, may be associated with stress ulcer (SU) development; a potentially fatal condition whose pathophysiology is still obscure. 2 Less severe, prolonged stress has been implicated in the pathogenesis of peptic ulcer disease 3 and irritable bowel syndrome (IBS). 4 appear to be interrelated and to be associated with affective and anxiety disorders.…”

“…[17][18][19] Moreover, a recent study has negated the involvement of H. pylori in stress ulcer bleeding in critically ill patients. 2 The pivotal role of the brain in the pathogenesis of stress-induced gastric mucosal damage has been well established. Increased vulnerability to depression 20 and anxiety 21 in experimental animals is paralleled with ulcer development.…”

“…Bleedi ng from stre ss ulcers in acutely ill children has been seen in associati on with acute respiratory failure, coagulopathy, and Pediatric Risk of Mortality Score of 1 0 or higher [2] and not report ed in cases of appendicitis. Studies for Helicobacter pylori infection were not conducted because it does not seem to play a pathogenic role in stre ss ulcer bleedi ng [11].…”
