Lower limb surveillance following autologous vein bypass should be life long

Abstract: The incidence of vein graft stenosis decreases significantly 1 year after operation but there is still at risk of developing potentially graft-threatening arterial stenoses. Legs that have undergone infrainguinal vein bypass grafting should continue to be monitored in a surveillance programme for life.

“…The need for frequent duplex examination of LEVG has been demonstrated in numerous studies. [1][2][3][4][5][6][7][8]14 With continued surveillance after the initial revision, some grafts will develop additional lesions and require multiple revisions. In this study, we have presented our experience with 37 patients who have undergone 50 secondary graft revisions.…”

“…The length of follow-up in such programs is controversial; both Idu et al 20 and Mills et al 21 suggest that as stenoses occur, early surveillance need only be performed for the first 6 months, whereas others recommend a longer follow-up. 22 The Transatlantic Consensus states that the optimum length and frequency of follow-up are unknown. 23 …”
