Lower Urinary Tract Infection and Periprosthetic Joint Infection after Elective Primary Total Hip Arthroplasty

Abstract: PurposePeriprosthetic joint infection (PJI) after total hip arthroplasty (THA) is a grave complication. Urinary tract infection (UTI) as a source for PJI is controversial. Our purposes were, (1) to evaluate the incidence of PJI after elective primary THA and (2) to determine whether UTI was associated with a risk of PJI after elective primary THA.Materials and MethodsWe retrospectively reviewed the medical records of 527 patients who underwent elective primary THA by using universal aseptic technique from May … Show more

“…Treatment of ASB detected on routine pre-operative urine cultures was responsible for 10/176 (5.7%) of inpatient antimicrobial prescriptions in the pre-survey, and 4/192 (2.1%) of inpatient antimicrobial prescriptions in the post-survey (-3.6%, p = 0.071). This treatment is not associated with reduction in operative infections after prosthetic joint surgery and is inappropriate [ 19 , 20 ].…”

“…On the other hand, Park et al 6) compared the incidence of PJI in a UTI group (13 patients) and non-UTI group (514 patients), and observed no significant association between preoperatively treated UTI and PJI. They treated UTI patients with antibiotics before THA if the urine culture was positive.…”

“…A diagnosis of urinary tract infection (UTI) is made when more than 10 5 CFU/mL is observed in urine culture 1) . Women are significantly more prone to UTIs than men; the reported prevalence of preoperative UTI in female patients undergoing total joint arthroplasty (TJA) ranges from 5.1% to 36% 2 3 4 5 6 7) .…”

“…An enlarged prostate may compress the urethra and cause anatomic bladder outlet obstruction, which may predispose patients to urinary retention requiring catheterization, urinary tract infection (UTI), and urosepsis [2,18]. UTI-associated organisms may then seed the implant via transient bacteremia, although this association is controversial [10,15,16,19]. As PJI is associated with high morbidity, such as persistent joint pain and function [22], and it is one of the leading causes of revision THA [9], exploring the association between sBPH and medical and implant-related complications may be beneficial in assessing whether perioperative assessment and management of patients with sBPH should be changed to improve surgical outcomes.…”
