Integrative and Analytical Review of the 5-Alpha-Reductase Type 2 Deficiency Worldwide

Abstract: The conversion of testosterone into dihydrotestosterone is catalyzed by the 5αreductase type 2 enzyme which plays a crucial role in the external genitalia virilization. It is encoded by the SRD5A2 gene. Allelic variants in this gene cause a 46,XY DSD with no genotype-phenotype relationship. It was firstly reported in the early 70s from isolated clusters. Since then, several cases have been reported. Putting together, it will expand the knowledge on the molecular bases of androgen milieu. Methods: We searched f… Show more

“…Even in the absence of dihydrotestosterone, the presence of androgens usually directs towards male psychosexuality development. This could explain the relatively high rate of gender change from female to male in 5 α RD2 deficiency, estimated 56–63% [ 18 , 19 ]. Because our patient was diagnosed early and was raised as a male, the conflict in identifying sex will be avoided.…”

“…DHT plays very important roles in mammalian sex differentiation as the most potent androgen ( 4 ). Mutations of the SRD5A2 gene cause 46, XY disorders of sex development, resulting from low DHT production ( 4 , 8 ). In contrast, little attention has been received in the existence and functions of DHT in fish, because it is thought to be relatively biologically inactive.…”

“…SRD5A2 is strongly expressed in male tissues, including prostate and epididymis for producing DHT during development (4). Therefore, mutations of the SRD5A2 gene cause 46, XY disorders of sex development, resulting from low DHT production (5)(6)(7)(8). SRD5A1 mainly plays roles in the production of neurosteroids involved in anxiety and sexual behavior (4).…”

“…A recent interesting review on SRD5A2 mutations found that exon 1 and 4 accounted for about 58% of all mutations reported in this gene and that five hotspots 55, 65, 196, 235 and 246 accounted for up to 25% allelic variants (Batista & Mendonca, 2020). G196S has emerged as a mutation hotspot in the SRD5A2 gene and has been described in Greek, Turkish, Swedish and Brazilian, American, European and Asian populations (Batista & Mendonca, 2020). This mutation has been shown to result in a consistent phenotype irrespective of the population where it was reported.…”
