Quantification of inaccurate diagnosis of COPD in primary care medicine: an analysis of the COACH clinical audit

Abstract: Background: Inaccurate diagnosis in COPD is a current problem with relevant consequences in terms of inefficient health care, which has not been thoroughly studied in primary care medicine. The aim of the present study was to evaluate the degree of inaccurate diagnosis in Primary Care in Spain and study the determinants associated with it. Methods: The Community Assessment of COPD Health Care (COACH) study is a national, observational, randomized, non-interventional, national… Show more

“…Finally, it also impacts on the health system leading to potential extra costs [29]. The reasons behind this high proportion or inaccurate diagnosis are multifactorial, where a low perception of symptoms among the population [30] combined with limited resources [31] and low awareness among physicians [32] combine to give rise to a perfect storm.…”

“…Firstly, although the starting sample size is large, the number of patients receiving monotherapy is more limited, which results in decreased statistical power and some potential associations not reaching statistical significance. The advancement of new treatments for COPD that provide greater bronchodilator and anti-inflammatory power, as well as the combination of several drugs in a single inhalation device, has led to single therapies being used less often [ 14 , 15 , 16 , 17 ]. In reality, this attitude towards stepping-up therapy has been widely adopted, although it may not be the most suitable approach, as we point out in our work.…”

“…Globally, 10 -95% under-diagnosis and 5 -60% overdiagnosis of disease are prevalent due to the unavailability of spirometry in rural areas of the developing countries, untrained technical operators to perform the test, and their lack of skill in interpretating the results of spirometry [12,13]. The diagnosis of COPD can only be declared if patients have a combination of certain symptoms, "a post-bronchodilator fixed ratio of Forced Expiratory Volume in 1 st second and Forced Vital Capacity, i.e., FEV1/FVC < 0.7, measured by spirometry [14]. Hence in the absence of any reliable and accurate diagnostic criteria, there is a dire need to search for some authenticated markers for the diagnosis of COPD.…”
