Risk Assessment of Recurrence and Autoimmune Disorders in Kikuchi Disease

Abstract: Purpose: Kikuchi disease (KD) is typically a benign disease. Recent studies reporting recurrence or serious cases suggest a possible association of KD with systemic autoimmune disorders. We performed a long-term analysis of the characteristics of KD in patients of all ages and assessed KD recurrence or progress to systemic autoimmune disorders. Patients and Methods: Electronic medical records of patients diagnosed with KD between April 1995 and May 2017 were reviewed for clinical and laboratory manifestations.… Show more

“…In a risk assessment study on KFD recurrence, patients with recurrent KFD had significantly more extranodal symptoms, higher prevalence of lymphopenia, and longer durations of lymphocyte count recovery. 28 Another study revealed that extranodal involvement and a long duration of symptoms were significant predictors of KFD recurrence. ANA positivity was more prevalent in the recurrent KFD group in that study, which is also consistent with our observations.…”

“…9 Patients with KD showing progression to autoimmune diseases were more likely to have fever, common extranodal symptoms, a higher recurrence rate, and a higher ANA positivity rate at KD diagnosis. 3 Hepatosplenomegaly was present in 18% of patients of KD 9 with few cases having clinically diagnosed AIH. Our case had biopsy proven AIH without clinical and laboratory findings of SLE.…”

“…The extranodal symptoms of KD are uncommon and diverse including skin rash, night sweats, weight loss, headache, cough, and abdominal pain. 3 The disease resolves spontaneously in weeks to months, but in some cases, it may have systemic involvement or evolve to SLE. 1 , 2 , 4 , 5 , 6 A definite diagnosis of KD is based on characteristic pathologic findings on biopsy that differentiate this disease from others such as lymphoma, and infectious lymphadenopathy.…”

“…Anti-DFS 70 autoantibodies were reported to be more prevalent in healthy individuals than those with autoimmune diseases such as systemic lupus erythematosus (SLE). In a long-term analysis of KFD patients, Hyun et al found that patients who developed autoimmune diseases after KFD were more likely to have extranodal symptoms, KFD recurrence, and anti-nuclear antibody positivity [27]. There is no current literature illustrating how best to follow KFD patients regarding long-term complications and disease associations such as SLE.…”
