Purposes: MicroRNAs (miRNAs) and their single nucleotide polymorphisms may be involved in the pathophysiological process of acute ischemic stroke (AIS), of which miRNA-146a is one of the most concerned miRNAs. This experiment is aimed to investigate the association of miRNA-146a and its single nucleotide polymorphism (SNP) with AIS and its susceptibility in a Chinese population. Methods A case-control study including 137 AIS patients and 100 controls were enrolled. The relative miRNA-146a expression in PBMCs was detected by real-time reverse transcription-polymerase chain reaction (RT-qPCR). And the SNP of miRNA-146a rs2910164 was genotyped using DNA extraction kit and TaqMan-MGB probe real-time PCR, and its relevance to AIS susceptibility was evaluated. Results The relative miRNA-146a expression in the experimental group (1.65 ± 0.11) was significantly higher than that in the control group (1.13 ± 0.09, P = 0.002). In subgroup analysis, the relative expression level of miRNA-146a in AIS with a course longer than 3 days was significantly higher than that less than or equal to 3 days and the control group (P < 0.05). Difference of the distribution of allele frequencies of the rs2910164C/G polymorphism was failed to found between the experimental and control groups (P = 0.703, OR = 0.930, 95% CI = 0.641–1.349). However, the GG genotype frequency was higher in AIS patients with other causes (SOE) than that in controls according to the TOAST subtype analysis. (P = 0.00, OR: 4.825, 95% CI: 2.720–8.562). Conclusions The findings suggested that miRNA-146a may be involved in the pathophysiological process of AIS. But the miRNA-146a rs2910164C/G polymorphism may not be associated with AIS genetic susceptibility, although the rs2910164GG genotype may increase the risk of SOE AIS.

miRNA-146a, a single-stranded, non-coding RNA molecule, has emerged as a valuable diagnostic and prognostic biomarker for numerous pathological conditions. Its primary function lies in regulating inflammatory processes, haemopoiesis, allergic responses, and other key aspects of the innate immune system. Several studies have indicated that polymorphisms in miRNA-146a can influence the pathogenesis of various human diseases, including autoimmune disorders and cancer. One of the key mechanisms by which miRNA-146a exerts its effects is by controlling the expression of certain proteins involved in critical pathways. It can modulate the activity of interleukin-1 receptor-associated kinase, IRAK1, IRAK2 adaptor proteins, and tumour necrosis factor (TNF) targeting protein receptor 6, which is a regulator of the TNF signalling pathway. In addition, miRNA-146a affects gene expression through multiple signalling pathways, such as TNF, NF-κB and MEK-1/2, and JNK-1/2. Studies have been carried out to determine the effect of miRNA-146a on cancer pathogenesis, revealing its involvement in the synthesis of stem cells, which contributes to tumourigenesis. In this review, we focus on recent discoveries that highlight the significant role played by miRNA-146a in regulating various defence mechanisms and oncogenesis. The aim of this review article is to systematically examine miRNA-146a’s impact on the control of signalling pathways involved in oncopathology, immune system development, and the corresponding response to therapy.

Stroke accounts for the second leading cause of death and a major cause of disability, with limited therapeutic strategy in both the acute and chronic phases. Blood-based biomarkers are intensively researched and widely recognized as useful tools to predict the prognoses of patients confronted with therapeutically limited diseases. We performed a systematic review of the circulating biomarkers in IS patients with prognostic value, with a focus on microRNAs and exosomes as predictive biomarkers of motor and cognitive recovery. We identified 63 studies, totalizing 72 circulating biomarkers with prognostic value in stroke recovery, as follows: 68 miRNAs and exosomal-miRNAs being identified as predictive for motor recovery after stroke, and seven biomarkers being predictive for cognitive recovery. Twelve meta-analyses were performed using effect sizes (random-effects and fixed-effects model). The most significant correlation findings obtained after pooling were with miR-21, miR-29b, miR-125b-5p, miR-126, and miR-335. We identified several miRNAs that were correlated with clinical outcomes of stroke severity and recovery after ischemic stroke, providing predictive information on motor and cognitive recovery. Based on the current state of research, we identified serum miR-9 and neutrophil miR-29b as the most promising biomarkers for in-depth follow-up studies, followed by serum miR-124 and plasma miR-125b.
