Objective To compare disability changes measured with the Respiratory ICF Maugeri core set on COPD patients, recovering from acute exacerbation with and without hospitalization, submitted to pulmonary rehabilitation (PR). Materials and methods All COPD inpatients admitted for rehabilitation in 9 Respiratory Units (January–August 2019) were considered eligible. 2066 patients were included (540 discharged from an acute Hospital = Hospital group and 1526 coming from their home = Home group). Healthcare professionals filled, in a digitalized chart, the Respiratory ICF Maugeri core set (26 items), assessing ICF categories at admission and discharge. Results The baseline distribution of the more severe ICF qualifiers was higher in the Hospital group ( p < .001) when compared to the Home group. After rehabilitation, all patients -irrespective of hospitalization need- statistically decreased the rate of the higher ICF qualifiers ( p < .0001). Hospital group improved more both the rate of qualifiers ≥2 [Δ: −21.32 (22.41) vs −15.48 (17.32), p < .001] and the rate of qualifiers 0–1 [Δ: + 18.38 (24.67) vs 13.25 (19.13), p < .001] than Home group. Conclusions Disability measured with the “Respiratory ICF Maugeri core set” after PR improves in COPD patients recovering from acute exacerbation irrespective of hospitalization need. Its use an additional outcome remains to be further elucidated. KEY MESSAGES Routine implementation of an ICF set for chronic respiratory diseases can enhance a patient-centered approach in rehabilitation for different severity conditions. Pulmonary rehabilitation (PR) seems to improve global disability measured with the Respiratory ICF Maugeri core set in COPD patients recovering from acute exacerbation irrespective of hospitalization need, suggesting the use of ICF set as additional PR outcome. The description, through the ICF language, of rehabilitative needs of patients, coming "from-Home" and "from-Hospital" settings, could help staff and instrument organization.

Or use: https://youtu.be/EP14t95RULYPurpose: To analyze the reliability and validity of the Brief international classification of functioning, disability and health (ICF) core set for chronic obstructive pulmonary disease (COPD).Patients and Methods: A cross-sectional study was conducted in four tertiary hospitals in Tianjin, China. A total of 100 patients with COPD were selected to evaluate functioning and disability involving body functions, body structures, activities and participation as well as environmental factors of the Brief ICF core set for COPD. Internal consistency was calculated by Cronbach's α. Content validity was examined using the content validity index (CVI), scale-level CVI/universal agreement, and scale-level CVI/average agreement (S-CVI/Ave).In addition, construct validity and convergent validity were also examined.Results: The Brief ICF core set for COPD had a high internal consistency, 0.873 for the total scale, with values of 0.750, 0.640, and 0.843 for body functions, body structures, and activity and participation, respectively. The content validity was calculated by the CVI, scale-level CVI/universal agreement, and S-CVI/Ave at values of 0.80-1, 0.929, and 0.986, respectively. Meanwhile, the ICF core set for COPD had good convergent validity, correlating with the mMRC dyspnea score (r=0.690, P<0.01), and there were significant correlations between the ICF core set for COPD and COPD clinical severity grade (r=0.363, P<0.01). A four-factor model of functions and disability in the Brief ICF core set for COPD had the best fit according to confirmatory factor analysis (CFA). Conclusion:The Brief ICF core set for COPD is a reliable and valid convenient instrument for assessing comprehensive problems in the functioning of patients with COPD, which can be used to design and to evaluate rehabilitation strategies.

BackgroundThe International Classification of Functioning, Disability and Health (ICF) offers a standardized international terminology to operationalize function management across multiple domains, but the summary score of the ICF qualifier scale provides limited information on the comparison of personal abilities and functioning difficulties.ObjectivesTo enhance the interpretative power of the ICF‐based Health‐oriented Personal Evaluation for the community‐dwelling older person (iHOPE‐OP) scale through the implementation of the item response theory (IRT) modelling.MethodsThis cross‐sectional, multi‐centre study administrated 161 ICF categories (58 on body functions, 15 on body structures, 60 on activities or participation and 28 on environmental factors) to evaluate the functional level of 338 older citizens (female = 158, male = 180) residing in community or supportive living facilities. The validation process encompassed assessing the IRT model fitness and evaluating the psychometric properties of the IRT‐derived iHOPE‐OP scale.ResultsThe age of participants ranged from 60 to 94.57, with the mean age of 70. The analysis of non‐parametric and parametric models revealed that the three‐parameter logistic IRT model, with a dichotomous scoring principle, exhibited the best fit. The 53‐item iHOPE‐OP scale demonstrated high reliability (Cronbach's α = 0.9729, Guttman's lambda‐2 = 0.9749, Molenaar‐Sijtsma Statistic = 0.9803, latent class reliability coefficient = 0.9882). There was a good validity between person abilities and the Barthel Index (p < .001, r = .83), as well as instrumental activities of daily living (p < .001, r = .84).ConclusionsIRT methods generate the reliable and valid iHOPE‐OP scale with the most discriminable and minimal items to represent the older person's functional performance at a comprehensive level. The use of the Wright map can aid in presby‐functioning management by visualizing item difficulties and person abilities.Implications for practiceConsidering the intricate and heterogeneous health status of older persons, a single functional assessment tool might not fulfil the need to fully understand the multifaceted health status. For use in conjunction with the IRT and ICF framework, the reliable and valid iHOPE‐OP scale was developed and can be applied to capture presby‐functioning. The Wright map depicts the distribution of item difficulties and person abilities on the same scale that facilitates person‐centred goal setting and tailors intervention.
