Lumbar Gout Tophus Mimicking Epidural Abscess with Magnetic Resonance Imaging, Bone, and Gallium Scans

Abstract: Gout is a common metabolic disorder, typically diagnosed in peripheral joints. Tophaceous deposits in lumbar spine are a very rare condition with very few cases reported in literature. The following is a case report of a 52-year-old patient with low back pain, left leg pain, and numbness. Serum uric acid level was in normal range. magnetic resonance imaging, bone scan, and gallium-67 images suggested an inflammatory-infectious process focus at L4. After a decompressive laminectomy at L4–L5 level, histological … Show more

“…To our knowledge, only a few case reports have described the use of SPECT/CT to evaluate gout tophi. 3 6 In these reports, anomalous increases in [ 99m Tc]-MDP were reported in tophi; these increases were presumably the result of inflammation, which led to greater blood supply in the region. Furthermore, tophi can cause bone erosion and remodeling.…”

“…To our knowledge, there have been few reports concerning the use of [ 99m Tc]-methylene diphosphonate (MDP) single-photon emission computed tomography (SPECT)/CT imaging to assess spinal tophi. [3][4][5][6] Here, we discuss the usefulness of [ 99m Tc]-MDP SPECT/CT imaging in the diagnosis of gout spondylitis.…”

“…Usually, this pattern of intraspinal gout tophus is confused with other spinal canal space-occupying lesions, such as abscess, disc migration, neoplasms, hematoma, and synovial cysts [3,7,8] . In the initial care plan, routine radiography is appropriate and cost-effective.…”

“…Usually, this pattern of intraspinal gout tophus is confused with other spinal canal space-occupying lesions, such as abscess, disc migration, neoplasms, hematoma, and synovial cysts. [3,7,8] In the initial care plan, routine radiography is appropriate and cost-effective. Although this preliminary screening tool lacks sensitivity, [9] it can identify lumbar instability associated with calcified gout.…”

“…Gouty tophus has been shown to be moderately hypermetabolic on fluorodeoxyglucose positron emission tomography CT (FDG PET CT) [ 85 , 86 , 87 ]. Tophaceous gout in the spine will show uptake on Gallium-67 imaging [ 88 ].…”
