Lumbar spine chordoma

Abstract: Chordoma is a rare tumor arising from notochord remnants in the spine. It is slow-growing, which makes it difficult to diagnose and difficult to follow up after treatment. Typically, it occurs in the base of the skull and sacrococcygeal spine; it rarely occurs in other parts of the spine. CT-guided biopsy of a suspicious mass enabled diagnosis of lumbar spine chordoma.

“…Lesions are usually well-circumscribed due to slow growth and may include calcification and marginal sclerosis (Hatem, Maclean). On T1-weighted MRI, chordomas appear as a low to intermediate-intensity lesion that may extend into or encase the adjacent neurovascular structures [ 7 , 8 ]. On T2-weighted MRI, chordomas appear as a high-intensity lesion, although the exact reason is unclear.…”

“…On T1 chordomas may show intermediate to low grade intensity, with hyperintense signal on T2 [Figures 3 and 4 ]. [ 29 ] Imaging features of chordoma and chondrosarcoma may appear similar, as both are hyperintense on T2. In one of our case of chordoma of odontoid, the tumour mass was causing cord compression.…”

“…The surgical goal for chordomas is centered on total en-bloc resection with canal decompression and spinal reconstruction as needed. [ 4 ] They primarily affect patients older than 40 years of age but have been observed in all age groups. [ 6 ] The 5-year survival rate for lumbar chordomas reaches 50%, with better prognoses correlating with early detection and gross total tumor resection.…”
