Lumen-apposing metal stents in interventional endoscopy: a state-of-the-art review with focus on technical and clinical successes and complications

Abstract: Endoscopic transmural drainage is considered the initial treatment option for pancreatic fluid collections. Recently, lumen-apposing metal stents have been introduced for pancreatic fluid collections drainage under endoscopic ultrasound guidance, in addition to evolving data in bile duct and gallbladder drainage. We aimed to perform systematic review with reporting pooled data analysis on technical success, clinical success and complications rate of endoscopic ultrasound-guided lumen-apposing metal stents uses… Show more

“…[ 16 17 27 ] Above all, what looks crucial to define is the adequate working setting, the essential need of a radiologic room, and a specialized team of interventional radiologists and pancreatobiliary surgeons. It is known, indeed, that LAMS placement has a not negligible rate of intraprocedural adverse events, such as technical failure, misdeployment, bleeding, and perforation[ 9 ] to manage whom fluoroscopy is needed, as well as radiological and surgical facilities.…”

“…In the last years, several studies have been published on the technical and clinical outcomes of LAMS[ 8 9 10 11 12 ] and their use and indication is expanding. To date, LAMS has three on-label indications: pancreatic fluid collection (PFC) drainage (PFCD),[ 13 14 ] secondary to acute pancreatitis, biliary drainage (BD)[ 15 16 ] for relieving biliary obstruction after failed ERCP, and gallbladder drainage (GBD)[ 17 ] for treatment of acute cholecystitis in high-risk surgical patients.…”

“…The specific cause of this complication is not clear. Most of the buried stents were case reports, but in one recent review concerning complications of LAMS, occurrence rates of buried stent in PFC, bile duct and gallbladder were 0.07%, 0 and 0.59%, respectively ( 54 ).…”

“…Además, permite realizar la técnica "freehand", lo que disminuye la dificultad a nivel técnico del procedimiento. Sin embargo, debido a su morfología y calibre, no pueden utilizarse para el abordaje de la vía biliar intrahepática, restringiéndose su uso para el drenaje biliar por CDS 35 .…”
