Lungensequestration bei Bochdalek'scher Hernie

Abstract: Representation on a case of lung sequestration in combination with a hernia of Bochdalek in a nine weeks old infant. A high standing right diaphragm, diagnosed through the X-ray pictures, commanded thoracotomy as choice approach to the diaphragm. On the inspection, we found a Bochdalek hernia and as an incidental finding, on the same side, a partly extralobar, partly intralobar sequestration of the lung.

“…Late presentation can either be acute or insidious onset with respiratory or gastrointestinal symptoms. CDHs are known to be associated with other congenital lung anomalies, such as pulmonary sequestration [3] or cystic adenomatoid malformations of the lung. It is well documented, however, that many latepresenting CDHs may previously have normal chest Xrays [4].…”

“…Around the 6th week, the pulmonary arteries will start to develop. This is also the time that asequestration may develop, ifthe original systemic arte rial supply is not replaced by a pulmonary arterial supply (9). In the same period, the oesophagotracheal septum is formed.…”

“…lndeed, part of the definitive diaphragm is derived from the embryonie septum transversum, in which the primitive liver develops. Thus, although the association of diaphragmatic eventration and sequestration of the Jung is rare, we can assume a single fault in the embryonie development around the 6th week.Thoracotomy, repair of the diaphragm and resection of the lung sequestration are the treatment of choice (1, 2,8,9,10).…”
